I think it is common courtesy to come without smelling or bad breath, however, apparently it is not. I do provide mouthwash but with smokers or certain foods recently consumed, sometimes that isn't enough. And in this case, the mouthwash provided was not even used. He expressed he had not seen many providers which may explain his lack of ettiquette, although I happen to believe ettiquette and common sense go hand in hand.
